Follicular unit transplantation (FUT) is a hair restoration technique, commonly known as the strip method, where a patient’s hairs are transplanted in naturally occurring groups of 1 to 4 hairs, called follicular units. It’s a gold standard method in hair transplant procedures to date. 

In this method, a strip of skin around 25-30 cm is taken from the back of the scalp under local anesthesia and dissected into grafts. These follicle grafts are then implanted in the bald region. The harvested area i.e, the area from the strip removed is closed by suturing. The scar left will be a thin line and is not noticeable with a normal hairstyle.

After measuring your donor site density, a strip is designed of adequate length and width (not exceeding 2 cm) from behind one ear to behind the other i.e, from safe donor area. This strip is planned so that it gives an adequate number of grafts and also at the same time allowing easy closure.

DONOR SITE PREPARATION AND ANAESTHESIA

  1. Trimming is essential in the donor site to harvest the strip and the team member in charge of the preparation ensures trimming the hair to 2- 3 mm all over the scalp.
  2. The next step is to wash the scalp.
  3. The donor area strip length and width to be harvested are marked out based on the skin laxity tests and depending on the individual’s follicular density per square cm.
  4. Once the marking is done, local anesthesia is administered to the donor area, usually preferring field blocks in that area.
  5. The harvesting of the strip is done with the patient in prone posture following the standards of care with an IV line, monitoring of the vitals under aseptic conditions.
  6. The strip that is harvested is preserved in cold saline.
  7. Wound closure is done.

The strip is then sliced into small bits of tissues with rows of hair follicles in them meticulously. At the end, individual grafts are separated. Higher magnification is used to perform this important step.

Then the grafts will be transferred to the recipient site.  For inserting follicle grafts very tiny incisions are made into the area of concern, which will be determined by the angle. the direction of hair and density of the hair to be transplanted. Once incisions are done, the follicle grafts are inserted using minuscule forceps.

Hair Regrowth After FUT Transplantation

After 11–20 days the new implanted hair will shed. Now the new follicles are in their dormant(sleeping) stage. There will be a slow phase before hair growth is initiated. Complete growth can be achieved between 6-9 months and hair continue to grow until 12 –14 months.
